Directs the Secretary of Health and Human Services to contract with the Tiffany Somerset Bangle Sheriffs' Association to serve as the Secretary's project liaison and coordinator to: (1) notify eligible county law enforcement agencies of the project; (2) serve as a national information clearinghouse for the project; (3) provide technical assistance; and (4) collect and refer completed demonstration project applications to the Secretary for review and approval. The contract amount shall not be less than three percent of the funds available for the demonstration project ($500,000,000).

Specifies that applicants seeking to participate in the demonstration project submit an Elsa Peretti Open center cuff to the National Sheriffs' Association (or its designee) and that such application be referred to the Secretary of Health and Human Services for review and approval.Requires demonstration project applicants, if approved, to implement a program that assesses offenders with mental illness, diverts non-violent and other offenders to community-based programs, and provides pre-release and transitional services for inmates.Permits Tiffany 1837 bangle project sites to use reimbursement funds from within their annual allowable reimbursement allocation to develop assessment, diversion, and transitional services capabilities.Establishes a joint peer review committee with equal representation from the Center for Medicaid and State Operations and the National Sheriffs' Association to review and recommend applications to the Secretary for approval.Requires the Secretary to ensure balanced representation in the final selection and approval of demonstration project sites.

Directs the Secretary of Health and Human Services to provide an Tiffany Knots cuff report to Congress assessing the effectiveness of the demonstration project and another annual report on the mental health needs of inmates. While there have been arguments for broadening this measure to capture a wider population, we believe that it is more likely that a focused and time-limited demonstration project will get off the ground.
